Adding a questionnaire is an optional option to request specific information from your registered members.

Tip: Parents can add contact information by logging into their My Account. This information will be synced with their profiles and reports in Spordle ID. We suggest asking parents to update their information here instead of asking for contact information in a form of a questionnaire. |

Questionnaires Page
From the Catalog / Questionnaires menu, you will find a list of questionnaires created for that season.
From the search box, you can enter partial or full to filter the display on screen.
You can click on the directional arrows to sort by that column.
The list of questionnaires will be displayed.

Adding Questionnaires
Click on Catalog, Questionnaires, and + Add.

Questionnaire Name: Enter a name to your questionnaire (this will not be visible to the public).
Section 1 of 1: Enter a section name
Question: Add a question and enter your question
Type: Select the type of answer you want from the following choices:
Short Answer: Allow the participant to enter a short answer
Long Answer: Allow the participant to enter a longer answer
Checkboxes: Allows you to make multiple selections using checkboxes
Radio button: Allows you to make a single selection using a circle
Single selection: Allows you to select a single answer from a drop-down menu
Multiple selections: Allows you to select multiple answers in a drop-down menu
Date: Allows you to indicate a date within a given date range
Time: Allows you to enter a time within a given time range
Number: Allows you to enter a number within a specified range
Mandatory: By checking this box, you can define this question as mandatory. The participant will have to answer to continue his online registration.
Admin Only: Admin only questions are answered only by registrar’s in HCR. If you add an admin only question, do not make it mandatory.
Click Create.

You can add a description to a section and/or question. This new formatted field lets you add explanatory text to your sections and questions.

If you wish to make changes to your questionnaire, click on the questionnaire, make the changes, and click Save.

Note: It is possible to modify a questionnaire, even if it has already been used in online registration, without deleting the answers already collected for the previous questions. The new questions will be added to the member's profile, and the member can answer them in My Account. |

Questionnaires are imported from one season to the next, but you can make changes to your existing questionnaires and even add new questions.
