Note: The Game Incident Report menu will be displayed according to your roles and permissions.

List of Game Incident Reports

On the Game Incident Report home page, a list of all incidents that have been submitted will be displayed. You can search by entering complete or partial information in the search field. You can also sort by using the directional arrows in each column.

  • Incident number

  • Season

  • Incident submitted by

  • Home team

  • Away team

  • Status

    • Pending

    • Closed

    • Completed

    • Extended

    • Submitted

 

Search for an incident report

Different filters allow you to search for a specific incident report.

  • Season: enter the season in which the indicator occurred.

  • Penalty Type: select the type of penalty you are looking for from the drop-down menu.

  • Penalty: select the desired penalty from the drop-down menu.

  • Start and end dates: Enter the date ranges of the incident you seek.

Add a Game Incident Report

To add an incident report, click the Tasks menu and select Game Incident Report.

Then click on +Add in the top right-hand corner.

 

Game Info

Give details of the game in which the incident took place.

The * indicates a mandatory field.

  • * Division: select the division from the drop-down menu.

  • Class: select the class from the drop-down menu.

  • * Category: select the category from the drop-down menu.

  • * Date: Enter the date of the incident.

  • League: indicate the name of the league.

  • Location: enter the location where the incident took place.

  • Game number: enter the game number.

  • Note: If necessary, add a note unrelated to the incident description.

    • For example, you could write that the game did not end

Click on Next.

 

Teams

Indicate the information for both teams.

The * indicates a mandatory field.

  • * Branch: select your branch from the drop-down menu

  • * Organization: select your organization from the drop-down menu

  • * Team: select the team from the drop-down menu

Note: If you can't find the team, click on the Other tab to add the information manually. However, one of the two teams must exist.

  • Coach: select the coach from the drop-down menu

Note: If the coach is not listed, select Non-Listed Coach and enter his first and last name.

  • Manager: select the manager from the drop-down menu

Note: If the manager is not listed, select Non-Listed Manager and enter the first and last name.

Click on Next.

 Officials

Add game official information by clicking on Add Official.

The * indicates a mandatory field.

  • *Official type: select the official type from the drop-down menu

  • * Email: enter the official's email address

  • * Phone number: enter the official's phone number

If you wish to add an official, click on Add an official.

Click on Next.

 

Incident

Add incident information.

The * indicates a mandatory field.

  • * Player: in the drop-down menu, select whether the player is from the home or visiting team.

  • * Player's name: select the player from the drop-down menu.

Note: If the player is not listed, select Non-Listed Participant and enter the required information.

  • Jersey number: enter the player's jersey number.

  • * Penalty Type: select the type of penalty from the drop-down menu.

  • * Penalty: select the penalty from the drop-down menu.

Note: if the incident is not related to an infraction, you must select Other in the penalty type box and select Other incident types in the penalty box.

  • Period: select the period in which the incident occurred from the drop-down menu.

    • If the incident occurs before or after the game, you must select Before or After the game and do not enter a time (minutes and seconds).

  • Minutes: enter the minutes of the game where the incident occurred.

  • Seconds: enter the seconds of the game where the incident occurred.

  • Description: enter a detailed description of the incident.

  • Sustained and injury: check the box if a player has been injured.

  • Add another incident: if another incident occurred during this game, click Add incident.

Click on Confirm

 

Submit Report

You will be asked whether or not you wish to submit the report now.

Select your choice, and the report will appear in your incident report list.

When you submit an incident report, you can select the email addresses you wish to use from your distribution list.

Each time you submit or resubmit a report, the system will display this window:

Game Incident Report side panel

If you click on one of the reports, a side panel will open to your right.

By clicking on the Action button, the following two options will be displayed:

  • Export to PDF

  • Resubmit the report

Three tabs will be displayed in the side panel:

  • Info

  • Teams

  • Incident

 

Info tab

This tab displays general information about the report. You can make changes by clicking on the blue pencil.

  • Report update date

  • Status

  • Incident date

  • League

  • Location of incident

  • Game number

  • Note

  • Names of officials

 

Teams tab

This tab displays information on both teams involved in the incident. You can make changes by clicking on the blue pencil.

  • Name of the home team

  • Name of home team coach

  • Name of home team manager

  • Away team name

  • Away team coach's name

  • Away team manager's name

 

Incident Tabs

This tab will display the incident details. You can make changes by clicking on the blue pencil or add a new incident by clicking on Add incident.

  • Penalty Type

  • Penalty

  • Period and time of period

  • Description of incident

Distribution list

You must add the list of people you wish the incident report to be sent to.

Note: This process is hierarchical, so emails added to your parent organization will also be added to your distribution list.

 

Add a member to the distribution list

To add your list of people, click on the Distribution list tab and click on Add.

 

Enter the first name, last name, email address, and status of the member you wish to add.

Note: if a member's status is inactive, it will not receive incident reports.

Click Add, and the member will be added to your distribution list.

Distribution list side panel

Changing members who have been added to your distribution list is possible. Click on a member's line to open the side panel.

You can then make the desired changes using the blue pencil.

If you wish to delete a member from your distribution list, click the Action button and select Delete.
